The 5th Avenue Theatre partners with local non-profits to invite underserved members of our community to our productions. Musicals and live theater are a vital part of our culture and we welcome all of our neighbors into our theater.
To request tickets for your organization, please fill out this form completely. Please submit only one application per organization. Your organization will be contacted as soon as specific Community Partners ("A Night Out") performances have been identified for an upcoming show.
PLEASE NOTE: Tickets are allotted based on availability; completion of this form does not guarantee tickets.
TERMS AND CONDITIONS: Please review the terms and conditions and sign below to indicate agreement to comply.
1. Ticket recipients will be families, individuals, and volunteers associated with your organization and otherwise unable to afford admission to a 5th Avenue production. (Execeptions may be made for chaperones of minors or other necessary staff.)
2. As donated tickets have monetary value, the partnering organization will make every possible effort to ensure that each donated ticket is used. Unused tickets may lead to forfeiture of program privileges.
3. Each recipient should be no younger than age 4, in compliance with 5th Avenue policy. No "babes in arms" will be permitted.
4. Organizations requiring special accommodations or ADA access will communicate specific needs to CAP program coordinator in advance so that Front of House staff may be properly advised.
