This year returnee employees may submit their own application if confident to do so (this must be done by the 16th of November), otherwise you may contact Chriz to organise a time to meet her on the 7th or 8th of November for her to submit them for you.

You will need to complete a visa application form (INZ 1142 Form). You can find this here.

You will not need to do a full medical or provide a good conduct certificate. You will need to apply for a new chest x-ray in the following circumstances: If your x-ray certificate is more than three years old, if it is less than three years old but you have not used it to come to New Zealand in the past or if you have spent more than six consecutive months in any country not listed on table one, page seven of the INZ1144 since your last one was issued. This means you will need to apply for a new one if you have been in Malaysia for 7 or more months since your last one was issued.

You will need to have your x-ray booked as soon as possible. Leaving it may mean not getting the results back in time. After your examinations you will be provided with an information sheet and NZER number. You will need this for your application.


You need to submit your original passport with your application. You will also need to submit two passport-size photographs of yourself no less than six months old, 2 photocopies of the bio-data page of your passport and a photocopy of every page with stamping and visa issued on your passport (Please do not include empty pages). Your application will need to be submitted in a white envelope (Size:229 x 324 mm).
