By signing and submitting this agreement, you authorize Provision Chartered Professional Accountants Inc., hereby known as Provision CPAs, to begin deductions for regular recurring payments and/or one-time payments from time to time, for payment of all charges arising under your account(s) with Provison CPAs.
Regular recurring payments for the full amount of services delivered will be debited to your specified account on the last day of the month. Provision CPAs will provide at least 5 days written notice of the amount of each regular debit. Provision CPAs will obtain your authorization for any other one-time or sporadic debits. Please note that the Personal T1 Tax Return is considered a regular debit on an annual basis.
The autority is to remain in effect until Provision CPAs has received written notification from you of its change or termination. This notification must be received at least ten (10) business days before the next debit is scheduled at the address provided in this agreement. For a sample cancellation form, or more information on your right to cancel a PAD Agreement, please contact your financial institution.
Provision CPAs may not assign this authorization, whether directly or indirectly, by operation of law, change of control or otherwise, without providing at least 10 daus prior written notice to you.
You have certain recourse rights if any debit does not comply with this agreement. For example, you have the right to receive reimbursement for any PAD that is not authorized or is not consistent with this PAD Agreement. To obtain a form for a Reimbursement Claim, or for more information on your recourse rights, please contact your financial institution.
